CBIP Management, Inc. will require the following underwriting information as
part of each submission.
- Truck application
- Small fleet application (40) units and under
- Large fleet application (41) units and over
- Supplemental Driver form, Supplemental Vehicle form, MVRS (No MVRs
- No quote)
- Large fleet - see requirements on last page of application
